If you are located within the state of Louisiana, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
- Establish overall housing strategy, execution and oversight of all housing initiatives and connection point to clinical and health economics team
- Actively network and builds relationships with senior staff at local public housing authorities, local continuum of care, state housing finance agency, and affordable and supportive housing owners and developers
- Develops systems and protocols to support members’ applications for housing assistance they are eligible for
- Serves as subject matter expert/authority on housing for all areas, including interaction with C-suite leaders and State Medicaid agency
- Actively engages with supportive housing providers and creates linkages for members to housing and supports
- Provides strategic consultation and thought leadership to influence and make recommendations to State and Local agencies/authorities for new program initiatives and innovation process improvements
- Represents the health plan to lead strategic initiatives with external housing agencies/providers focused on increasing access to affordable housing in the community
- Drives specific housing-related initiatives and support through collaboration with clinical teams to define, develop and monitor programs and impact on health outcomes, quality initiatives and cost impacts
- Creates linkages to support members in efforts to find and maintain stable housing through research and networking within communities; occasionally assisting members with housing applications
- Day to day monitoring, testing and assessments of pilots to reduce costs, improve quality outcomes and comply with state requirements
- Consults with other markets to share local initiatives and successes relative
Key Responsibilities:
- Service Coordination:
- Coordinate and manage the delivery of housing services, ensuring members receive appropriate support such as housing assistance, referrals, and case management
- Work closely with internal departments, providers, and community partners
- Program Implementation:
- Assist in the development and execution of housing and special program initiatives
- Ensure that all services and programs are delivered efficiently and effectively, meeting the needs of members
- Maintain up-to-date records of service delivery, member engagement, and outcomes
- Reporting and Documentation:
- Maintain accurate records and documentation related to housing services, volunteer activities, and special programs
- Prepare regular reports on service delivery, volunteer engagement, and program outcomes for review by supervisors and stakeholders
- Support and Collaboration:
- Work closely with other team members and departments to ensure a coordinated approach to service delivery
- Provide administrative support for program activities, including scheduling, communications, and logistics

Required Qualifications:
- 7+ years of experience in any of the following: affordable housing finance, community development, homeless services, supportive housing services
- Proven experience communicating effectively with members as well as Executive Leadership
- Understanding of the federal, state, and local laws and policies that govern housing and homeless programs
- Solid understanding and respect of all cultures and demographic diversity
- Intermediate working knowledge using Microsoft Office Programs specifically Teams, Excel, PowerPoint, Outlook and Word
- Demonstrated success working effectively in a highly matrixed and ambiguous environment with the ability to shift gears comfortable
- Proven track record in partnership development at all levels
- Proven exceptional communication & interpersonal skills with the ability to build rapport with internal and external customers and stakeholders
- Proven decision-making skills regarding own work approach/priorities, and work assignments, standards and resources
- Proven ability to multi-task and work in a very fast-paced environment
- Proven solid written communication skills and a solid advocate for members at all levels of care
- Ability to travel statewide
Preferred Qualifications:
- Experience in housing, program coordination, and/or community services
- Understanding of a matrix organization
- Proven solid organizational and multitasking skills
- Proven excellent communication and interpersonal abilities
