Legacy Management Group owns and operates skilled nursing facilities across Louisiana and in Texas. Starting pay is $110,000 based on experience.
General Job Summary:
The Nurse Practitioner functions within both the Skilled Nursing Facility setting and the long-term care environment providing primary care to residents. She/he works in collaboration with the facility's Interdisciplinary Team. The Nurse Practitioner demonstrates a strong foundation in evidence-based practice and education and encourages an environment that provides high quality, compassionate, patient-centered, safe care. The Nurse Practitioner must demonstrate a high level of medical and nursing knowledge that translates into technical and clinical competence and must exhibit professionalism in all interactions, whether residents, staff, or families.
Essential Job Functions:
- consults supervising attending physician as needed
- documents resident visits
- participates in quality improvement
- available on call as per schedule
- will review, approve, and modifies admission orders
- creates detailed admit note for each admission/ readmission
- initiates/ documents Advanced Directives
- review orders
- review labs, radiology reports, and consults
- visits skilled residents during rounds and documents
- assess residents' medical stability; consults/coordinates with specialists as needed
- addresses acute medical or mental status changes
- coordinates/assess rehab progress on a daily basis
- discusses concerns with the resident, family, rehab, and case management regarding illness, diagnosis, and treatment options
- attends care plan meetings when needed
- assists Medical Director in clinical management of the facility
Requirements:
- License to practice as a Nurse Practitioner issued by the State Board.
- Clinical experience, especially geriatric/ LTC experience preferred, but not required.
- Geriatrics specialty certification preferred, but not required.
