Job Summary
The LPN is to provide routine care, observe patients’ health, assist doctors and registered nurses, and communicate instructions to patients regarding medication, home-based care, and preventative lifestyle changes.
Job Responsibilities
- Administer direct patient services requiring specialized nursing skills.
- Effectively carries out physician's orders.
- Recognizes, observes, records and reports significant changes in the patient's condition to the supervising RN and Physician.
- Check vital signs and medication regime on assigned patients.
- Assist with feeding, ambulation and personal care.
- Provides education to the family, caregiver and patient on procedures and disease management processes specific to the patient's health care needs.
- Is courteous and maintains good rapport with the patient, caregiver, family and all other parties involved.
Qualifications
- Graduate from an accredited school of vocational nursing. Must have current LPN license with no restrictions.
- One year of nursing experience in which the principles and methods involved in patient care, orientation and in-service education were practiced is preferred.
- Must have current driver's license and auto insurance with a dependable mode of transportation.
- Must have current BLS CPR certification.
- Must have excellent oral & written communication skills.
